RN Case Manager


Hospice of the Carolina Foothills is seeking a full-time Registered Nurse Case Manager. This is a salaried full-time position with benefits.


  • Education: Associate degree in nursing required, Bachelor of Science in Nursing preferred, certification in hospice and palliative care preferred.
  • Experience: One year of nursing experience preferred.
  • Current North Carolina Registered Nurse license.
  • Valid driver’s license in good standing (driving record must meet HOCF requirements) and current auto insurance required.
  • Ability drive and travel regularly.
  • Excellent computer skills required.
  • Excellent verbal and written communication skills, problem-solving skills, logic and reasoning skills.
  • Excellent customer service skills.
  • Ability to maintain confidentiality, professionalism, and integrity at all times.
  • Understanding of and commitment to the hospice philosophy of care.
  • Ability to work in a team environment with volunteers.
  • Ability to identify and accept the needs of people as they differ according to social status, race, beliefs, and their emotional, physical and intellectual capacities.
  • Ability to work under pressure.
  • Flexibility and cross-training essential.
  • Ability to read, comprehend and write simple instructions, short correspondence, and memos. Ability to effectively present information in one-on-one and small group situations to patients/families/caregivers, and other employees of the organization.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
  • Physical requirements: Regularly required to: bend, stoop, stand, walk, use hands to fingers, handle or feel objects, use tools and/or controls, reach with hands and arms, talk and hear. The employee is occasionally required to: taste and/or smell, sit, climb, balance, stoop, kneel, crouch, and/or crawl. The employee must occasionally lift and/or move up to excess of 50 lbs. Ability to transfer patients in excess of 200 lbs. Specific vision abilities required by this job include: close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to use fine motor skills, sit for long periods and view a computer screen for long periods.
