Hospice of the Carolina Foothills is seeking an Intake/Admission Registered Nurse. This is a full-time position with a competitive salary and full benefits.

JOB SUMMARY

The Intake/Admission Registered Nurse will ensure the intake and timely processing of referrals to hospice and palliative care. This position will follow through the process to its completion with admitting these patients in their homes or facilities.

QUALIFICATIONS

  • Education/Experience: Has one of the following combinations of nursing degree and experience in a heath care setting (hospice or home care nursing experience preferred):
  • Associate Degree in Nursing (ADN) with at least two years’ experience; OR
  • Bachelor of Science in Nursing (BSN) with at lease one years’ experience.
  • Current multi-state Registered Nurse license (NC/SC) in good standing, certification in Hospice and Palliative care preferred (CHPN).
  • Valid driver’s license in good standing and current auto insurance required.
  • Ability to perform duties with a high sense of urgency, ability to multi-task and perform under pressure.
  • Excellent computer skills required.
  • Excellent verbal and written communication skills, problem-solving skills, logic and reasoning skills.
  • Excellent customer service skills.
  • Ability to maintain confidentiality, professionalism, and integrity at all times.
  • Ability to be flexible with schedule and work duties and to participate in cross-training as needed.
  • Ability to work independently as well as in a “team” with volunteers.
  • Ability to identify and accept the needs of people as they differ according to social status, race, beliefs, and their emotional, physical and intellectual capacities.
  • Thorough understanding of and commitment to the Hospice philosophy of care, reimbursement and certification issues needed.
  • Ability to maintain a high level of community visibility, especially with physicians, other health professionals and providers of service.
  • Ability to relate effectively with patients, physicians, community agencies and institutions.
  • Should be organized, innovative, flexible and able to communicate effectively and demonstrate the ability to manage effectively.
  • Physical requirements: Bend, stoop, lift a minimum of 25lbs., use fine motor skills, sit for long periods, view computer screen for long periods, and physical stamina to withstand constant activity.
